Can a family member order for me?
Orders can only be placed by you (the member). If you have an authorized representative on file, that person can order on your behalf. Funds are non-transferable.
Can I add my own dollars to the card?
No. You can’t add your own money to your Asuris Flex Card.
Can I get Healthy Rewards for a Flu Shot?
Healthy Rewards are no longer available in 2026; however, you may receive an incentive for the annual flu vaccine if you received it in 2025. There are a few ways to claim your incentive after you get your annual flu shot. If you received your shot in 2025 and in a medical office setting, your provider would bill a medical claim. Once the claim is received and processed, your reward will be loaded into your Asuris Flex Card. Allow 60 days for claims processing.
If you received the annual flu shot in 2025 from another provider that would not bill a medical claim (for example, a pharmacy, employer sponsored event, or health fair), you may tell us that you received your flu vaccine by calling Flex Card Services 1-866-228-7001 (TTY: 711), or by completing an attestation form online or in the myTotal Benefits app. You must have submitted your attestation in 2025.
The attestation form is available online only and not available on a paper form. The attestation form is only available during 2025. If you received your flu shot in 2025, and would like to attest and receive your reward, call Flex Card Services 1-866-228-7001 (TTY: 711)
Can I return items I ordered?
No, cancellations and returns are not accepted.
If you receive a damaged product, call Flex Card Services 1-866-228-7001 (TTY: 711). The product will be exchanged for an identical product at no cost to you.
Can I share items with my family members or friends?
No, items are for your use only. Using these funds to order items for anyone other than the member is prohibited.
Can I shop at the same stores for OTC and Healthy Rewards?

Do I have to pay for shipping?
Shipping is free when ordering online through the catalog portal at AsurisFlexCard.com, through the myTotal Benefits app, when ordering by mail, and when ordering on the phone.
Do the funds on my card expire?
Over the counter & home and bathroom safety device allowance
Yes, your OTC & home and bathroom safety device allowance expires at the end of each calendar quarter. Funds not used prior to midnight Mountain Time (MT) on the last day of the calendar quarter will expire.
First Quarter: January 1 – March 31
Second Quarter: April 1 – June 30
Third Quarter: July 1 – September 30
Fourth Quarter: October 1 – December 31
Healthy Rewards
Rewards earned during the 2025 calendar year will expire on March 31, 2026.
Does the Asuris Flex Card offer discounts above the allowances?
No, there are no additional discounts with the Asuris Flex Card.
How can I check my card balance?
You can check your card balance 24 hours a day, seven days a week. Just call 1-866-228-7001 (TTY: 711). Then select option 1. You will need to have your card number on hand.
You can also check your balance online. Log in to your account at AsurisFlexCard.com. Or use the
myTotal Benefits app on your smart device.
How can I spend my Healthy Rewards?
You can spend your rewards on pretty much anything at participating stores. Swipe your Asuris Flex Card at the register and select Credit to complete your purchase.
You will need to enter the PIN to make a debit purchase. You may skip the PIN requirement by selecting Credit at checkout.
Rewards cannot be used to buy alcohol, cigarettes, firearms, ammunition, lottery, pornography or prescription drugs.
How do I activate my card?
You must activate your card before you can use it.
1. Call 1-844-210-2175 (TTY: 711). You’ll be asked for your card number and the card expiration date.
2. Log in to AsurisFlexCard.com or the myTotal Benefits app. Click My Account. Then select Activate Card from the drop-down menu.
A PIN is required when activating your card for security purposes, however, there is no requirement to use your PIN when making purchases as credit.
How do I earn Healthy Rewards?
Visit AsurisFlexCard.com to view your qualified Healthy Reward activities.
Services must be completed by December 31, 2025, to be eligible for rewards.
Only one reward can be earned for each service per year. If multiple screenings are completed, your reward amount is based on the first claim we receive.
How do I get the myTotal Benefits app?
You can download the myTotal Benefits app in the App Store® or Google Play®. There’s no cost to download it. But you do need internet access. Data rates may apply.
You will use the same login info for both AsurisFlexCard.com and the app. If you don’t already have an online account, you can sign up for one through the app.
The myTotal Benefits app lets you check your card balances and order select wellness items. Plus, the app comes in handy when shopping for OTC items in person. You can use the product scanner to scan the barcode and see if items are approved to pay for it with your Asuris FlexCard.
How do I order from the OTC & home safety device catalog?
There are three easy ways to order from the catalog:
• Online: You can place an order online at AsurisFlexCard.com. Or you can order through the myTotal Benefits app on your smart device.

How do I sign up for my online account?
Signing up for your online account only takes a few minutes. Just go to AsurisFlexCard.com. Then click Sign Up on the Member Login screen. You will be asked for your:
• Member ID (enter your entire Member ID exactly how it appears on your Member ID card)
• Date of birth (enter it in this format: MM/DD/YYYY)
• Current email address – This is optional
• Current phone number (enter numbers only — no dashes) - This is optional
You will then be asked to create a unique username and password for your account. Once you have done this, you can log in 24/7 to:

Is there a limit to how many orders I can place?
No. There’s no limit on the number of orders that can be placed. You are only limited by the dollar amount in your allowance and to approved items.

What are Healthy Rewards?
With the Healthy Rewards program, you can earn rewards while taking care of your health.
Your rewards automatically load on your card throughout the year as you complete qualifying services. Allow up to 60 days from the date of your service for your claims to process.

What happens if an item is damaged during shipment?
If you receive a damaged item, please call Flex Card Services at 1-866-228-7001, (TTY: 711). This product will be exchanged for an identical product at no cost to you. Please note that only damaged items
can be exchanged within 30 days of purchase, no returns are allowed.
What happens if I mail in an order on the last day of the month?
We’ll deduct funds from your allowance balance based on the date when we receive your order. For example, if you mail your order form on March 31st and we get it on April 5th, we’ll deduct the funds from your April allowance.
If you’re ordering by mail and want to spend your current balance, be sure to send it well before the end of the quarter. If it’s close to the end of month, you can:
• Order online any time before midnight Mountain Time on the last day of the quarter
• Shop in person at a participating store any time before midnight Mountain Time on the last day of the quarter (remember to check the store’s hours)

What if I don’t have enough funds to cover a purchase?
When shopping the online OTC & home and bathroom safety device portal, during checkout, you can enter an additional credit card to complete your transaction.
When shopping in-store, if you don’t have enough funds on your card to make a full purchase, let the retailer know. Tell them how much to charge to your Asuris Flex Card. Then you can use another form of payment to cover the balance. If you don’t do this, your payment may be declined.
